Coding for Neuroma Injection

"Please provide a diagnosis code for an injection of Morton's Neuroma. Its a therapeutic injection not a trigger point injection. The injection is a combination of lidocaine and cortizone. An office visit code of 99213 was billed and paid but not the 20550."

The diagnosis codes for a painful neuroma are:

Primary Diagnosis:    G57.61 - Lesion of plantar nerve, right lower limb OR G57.62 - Lesion of plantar nerve, left lower limb  OR G57.63 - Lesion of plantar nerve, bilateral lower limbs

Secondary Diagnosis: M79.671 - Pain in right foot AND / OR M79.672 - Pain in left foot             

The CPT® code for injection of a neuroma is:

CPT 64455® - Injection(s), anesthetic agent and/or steroid, plantar common digital nerve(s) (eg, Morton's neuroma)

CPT® 20550 is for injection of "single tendon sheath, or ligament, aponeurosis (eg, plantar "fascia")" and should not be used to represent injection of a Morton's nueroma.

Lastly, the E&M should only be submitted with the injection if the E&M was significant in nature and separately identifiable from the procedure and this is supported in the documentation.
