What is the proper code to use for casting supplies?

What is the proper code to use for casting supplies? my biller has tried using code A4590 and 99070 and Q4050, which have all come back denied as being bundled into procedure code 29515 or 29405.

There can be some exceptions to this, but for the most part, the casting supplies associated with CPT® 29405 (Application of short leg cast (below knee to toes) are represented by these codes:

Q4037 - Cast supplies, short leg cast, adult (11 years +), plaster
Q4038 - Cast supplies, short leg cast, adult (11 years +), fiberglass
 
Q4039 - Cast supplies, short leg cast, pediatric (0-10 years), plaster
Q4040 - Cast supplies, short leg cast, pediatric (0-10 years), fiberglass
 
Code selection depends on if the cast is applied to a patient 0-10 years old or someone older.  Selection also depends on if the majority of the casting materials used was plaster or fiberglass.

Q4045 - Cast supplies, short leg splint, adult (11 years +), plaster
Q4046 - Cast supplies, short leg splint, adult (11 years +), fiberglass
 
Q4047 - Cast supplies, short leg splint, pediatric (0-10 years), plaster
Q4048 - Cast supplies, short leg splint, pediatric (0-10 years), fiberglass
 
Code selection depends on if the splint is applied to a patient 0-10 years old or someone older. Selection also depends on if the majority of the casting materials used was plaster or fiberglass.
