Injection for 1ST MP Joint-RT (Retrocalcaneal)

My patient has gout in the 1st MP joint right and the right retrocalcaneally.I gave an injection in each area.2 total.ICD: M10.9,M79.671,M79.676.How is this billed?

The retrocalcaneal injection would be coded with CPT 20605 since this would be considered an intermediate sized bursa. The first MPJ injection would be coded with CPT 20600 since this is a small joint. Both procedure codes would need an RT modifier since the injections were given on the right foot. The MPJ injection would need modifier -59 added so it does not deny inclusive to the other injection. Gout would be listed as the primary diagnosis, and pain would be listed secondary.
