Correct Billing for 11719, 11721 and 11056

CPT codes 11719 (nail trimming) and 11721 (nail debridement 6-10) should not be billed together as 11721 is already reimbursing for up to 10 nails and nail trimming would be considered bundled. If debriding (1-5) then the use of CPT 11720-59 may be billed with trimming CPT 11719.
