Medicare Denials-Missing/incomplete/invalid patient identifier

I have received several denials from Medicare stating " missing/incomplete/invalid patient identifier" These patients have been in before., claims have been paid and timing for Medicare visits is appropriate( 63 days or thereafter). Please advise

Due to certain data breaches that happened within the past year that caused Medicare beneficiaries’ personal information to be compromised, CMS has reissued new Medicare cards and ID numbers. It is very important that you check the patient’s insurance for every encounter due to changes in insurance, id numbers, etc…
