HealthPlus Pre-Authorization for Orthopedics Footwear Codes

Effective June 1, 2015, HealthPlus Amerigroup will require preauthorization approval for the orthopedic footwear codes (L3000 to L3630) based on guidelines set up by the New York State Medicaid Fee-for-Service DME Guidelines. For a complete list, go to page 141 of the provider manual at this link:
